If you are a new patient and do not complete your paperwork in advance through the patient portal please arrive 30 minutes prior to your appointment to complete it. This section is for people interested in becoming new patients at APG Health. Complete the form and we will contact you within 48 hours to schedule an appointment.

What is the new patient portal (powered by AdvancedMD)?

IMPORTANT ANNOUNCEMENT : Effective August 30th, we are starting with a new Patient Portal (powered by AdvancedMD) which will replace EPIC Mychart Messages. AdvancedMD will give you access to your personalized patient portal where you can leave a message for your provider for assistance or to view your medical bills.

When should you drive after an APG observation?

Additionally, all patients will require transportation from APG Healthcare following the observation period,as they should not drive or operate machinery until the day after a treatment session, following a restful sleep.
