If you or a family member were born at an Advocate hospital and need to obtain a copy of a birth certificate, contact the Illinois Department of Public Health.

Cost

If your medical records are being sent directly to a doctor or other health care providers, there is no fee. For all other types of requests, there may be a reasonable cost-based fee. If you have questions about fees associated with getting medical records, call 847-990-5250.

What time does Condell Cafeteria open?

Condell Cafeteria. Open weekdays from 6:30 a.m. to 1:30 p.m. Located on the lower level of the hospital, the Condell Cafeteria is a full-service facility that offers hot and cold meals—including hot breakfast, soups, salads, deli sandwiches, pizza, burgers and hot entrees—at various stations. How to contact labor and delivery?

All visitors to Labor and Delivery must sign in at the desk. Please call 847-990-5470 for more information. Any person in the labor and delivery room must have a visitor badge with the number of the labor room. The ID must be worn above the waist and clearly visible.

Where should a clergy check?

Clergy should check with guest services located at the front desk for the locations of patients they are to see. Clergy may visit patients anytime, except when the patient is receiving care. Clergy should check at the nurse's station if a notice states that a patient has visiting restrictions or is in isolation.
