Any printed forms will need to be brought in to the Austin Gastroenterology office at the time of your visit. The boxes below contain various forms for you to download, print, and fill out. The forms you need will depend on whether you're a new or existing patient. If you're having a procedure done, you may need to download a prep form.
If you would like to schedule a Telehealth Visit please consider the following: 1 Telehealth Visits require both audio and visual capabilities 2 These visits are scheduled at a specific time with your doctor with a few steps that must be completed PRIOR to the Telehealth visit such as filling out consent forms online and making sure the camera and microphone are enabled on your laptop, desktop, or smart device. 3 Like in-person office visits, patient deductibles and co-pays may apply, which vary according to each patient’s insurance plan. It is best that you check with your insurance provider directly as many carriers are adapting their policies day by day throughout this pandemic.

Telehealth Visits via Doxy .me are HIPAA compliant, simple and easy to use. There is no need for patients to download the software or create an account. Just use a browser on a computer or device with a camera and microphone. Telehealth Visits are private and secure.
Endoscopy Center I (North) The Center is located between Anderson Lane and Steck Avenue. It is directly across the street from Lowe’s. The Center is easily accessible from 183 and MoPac. There is a gray canopy extending over the entry and it is on the southwest corner of the complex.
Endoscopy Center II (South) The Center is located close to 290/71, directly off of the frontage road at First Street and James Casey Street. It is behind the South Austin Medical Center and located in the Garden Medical Center.
Located at the rear of the parking lot, it is the only 2-story building in the complex. Hours. Monday – Friday, 6 am – 4 pm. Services.
