The Patient Portal provides a secure way to get connected to your health information online. The Portal allows you view your lab work, receive notifications/health reminders from your provider, receive a summary of your health record, and much more!
To request your medical records, please fill out a HIPAA Form and email to email@example.com. You can also pick up and/or submit a form at any front desk. Please note this process can take 5-7 business days.
