What features are no longer available through CentraState patient portal?

Features in your CentraState portal account, including Appointment Requests, Medication Refills, Communications, and Statements, will no longer be accessible through your practice’s patient portal. We encourage patients who would like records of past communications with their physicians to download them prior to June 22.

732-294-2707 from outside the hospital OR Ext. 2707 from an in-house phone. CentraState’s care coordinators are certified nurses who support patients, their families, and other caregivers by coordinating and monitoring care from pre-admission to post-discharge from the hospital.
