Mission Statement. The mission of Chapa-De Indian Health is to advance the health and well-being of American Indians and low income individuals living in our communities by providing convenient access to high-quality, compassionate care.
Chapa-De has a healthcare model that is both patient-centered and integrated. The patient is the central focus of our services and processes, and they will receive care from a highly skilled team of healthcare professionals. Chapa-De’s main priority is to serve American Indians, Alaska Natives and low-income patients.
Our optometry department is located in our Auburn health center and offers eye exams for everyone in your family, starting with children as young as 4 years old. Chapa-De has an outstanding team of psychiatrists and therapists who want to help you meet your goals for mental and emotional health.
ACCESS | Appointments are provided within a reasonable time. You will be treated without discrimination as to race, color, age, sexual orientation or preference, gender, national origin, political beliefs or disabilities.
RESPECT | Follow rules including the consideration of the rights of other patients and staff. Please assist with the control of noise and observe the no-smoking policy.
All new patients must register before their first appointment, please see How to Register. New patients should arrive at the reception desk in the department you are visiting 30 minutes before your first appointment to complete the new patient registration process. Established patients should arrive 15 minutes before your appointment time.
Chapa-De is committed to protecting the confidentiality and privacy of health records and information. We want you to be informed about your rights to privacy as a patient.
Chapa-De offers a sliding fee scale for uninsured and low income patients depending annual income and family size. This discount fee scale is for Non-Indian patients, and is based on Federal Poverty Guidelines. We have a 50% or 75% sliding scale, which is the portion of the charges that the patient must pay.
Family PACT provides family planning services, such as birth control pills, IUDs, vasectomies, and education to low income men and women of child bearing age. While there is no charge to American Indians for care at Chapa-De, most services outside of our clinic are not offered free of charge. This includes services provided by medical specialists, hospital stays, emergency room visits and medical x-rays.
If you are injured, become ill or have health concerns or questions when our community health centers are closed (between 5 p.m. – 8 a.m. on weekdays or on weekends and holidays) you can still call us for help.
