How do I update my medical records with chpconnect?
After picking your physician, update your records either by calling Capital Health Plan Member Services or by making the selection in your CHPConnect account, which automatically sends a secure message to Member Services.
How do I get help enrolling in the new hospital portal?
For assistance with the new hospital portal or to obtain your Community Medical Record Number which will be needed to enroll, please contact the Health Information Management Department at 609-303-4085 (Monday through Friday, 8 a.m. - 4:30 p.m.).
How do I contact Cerner support for the new hospital portal?
For assistance with the new hospital portal after normal business hours, please contact Cerner Support at 866-550-7304 (4:30 p.m. - 8 a.m.). In addition, we recommend that you print copies of your records from the below portal in the event you are not able to access the portal in the future.
