As a Presbyterian Medical Group patient, you have secure online access to your Presbyterian electronic health records through your MyChart account. You can also have access to your family's records by requesting proxy access and MyChart offers teens, between the ages of 14-17, access to directly engage with their care team.
Connect is a joint effort of Columbia, NewYork-Presbyterian, and Weill Cornell Medicine, so a single Connect account links you to all three institutions. With the Columbia Connect app, you can access convenient services to help manage your health.

We are committed to providing the highest quality care to our patients. To this end, we are implementing Epic, a single health record system. Along with Epic, we are introducing our new patient portal — Connect — which will give you detailed, trustworthy information about your health.
To request a medical record, please visit Medical Records for more information.
With proxy access, you can view certain medical information of your children or other adults.
If you are a parent or guardian of a minor child under the age of 14 and would like to request proxy access to their medical records, you will be required to have your own MyChart account.
If you are an adult and would like to provide limited or full view access to your medical records, you can do so from your MyChart account. Once logged in, choose Share My Record from the menu.
MyChart is NOT to be used for urgent needs. Please visit one of our urgent care locations or call your clinic. For medical emergencies, dial 911.
On the myPRES Registration page, enter your information and select Register.
