The Health app on iPhone & iPod touch acts as one centralized hub for all your health data from various third-party apps and devices such as your Apple watch, run or step trackers, sleep trackers, food diaries, etc. Community Medical Center supports health records on the Health app for Apple iPhones & iPod touch devices.

Access standard information (last visit notes, medical history, medication, radiology and lab results) in your medical records through our patient portal.

2. Submit an Online Request Form

For standard records as well as records not available through the patient portal, use the fast and secure online electronic Patient Request app hosted by our release-of-information partner, Ciox.

Visiting Hours and Duration

All visitors must be 18 years of age or older, except in rare exceptions as determined by the facility.

Personal Protective Equipment (PPE)

All visitors will be required to wear a facemask. They may wear their own facemask/covering upon arrival to and throughout their stay in the facility, as recommended by the Centers for Disease Control and Prevention (CDC). If they do not have their own facemask/covering they will be provided one.

Temperature, Symptom and Travel Screening

All visitors must undergo symptom and temperature checks upon entering any of our facilities. If they present with fever or symptoms, they will not be allowed entry into the facility as recommended by the CDC.

Online Access to Your Personal Health Information

At the hospital patient portal, you can access read-only medical information through our secure server. This includes educational materials, information on your current medications, allergies, immunizations, health issues, and lab test results. All information is pulled directly from your electronic health records.
