Go to the MyDCH portal site and click on the "Sign Up" link beside "Don't have an account?" You will be asked to confirm your identity for the Portal to access your records. You will need to enter your Last Name, First Name, Date of Birth, Last 4 digits of your Social Security Number and the email address you provided.
If you provided your email to DCH staff during your visit, you will need to access the MyDCH portal page to complete your self-enrollment access request. Please use the following steps to complete this request:
