Access Patient Portal. Click Settings > Patient Portal . The Patient Portal Dashboard page launches in a web browser. An alternate way to access the Patient Portal is to: Open a web browser and type portal.kareo.com in the address bar. Click For Doctors on the bottom. The Patient Portal landing page opens. Click Sign in on the upper right.

If using a mobile device, you must switch to full site mode to access registration. 2. Go to https://emrmail.hallmarkhealth.org a. This will launch the Patient Portal. 3. New Users: Click on the green Register and Activate Now Button
The portal provides you with online access to your medical information on a convenient and secure site. View your personal health records, clinical summaries, laboratory and imaging results, as well as instructions and education specific to your care.

At your next visit, tell the staff you would like to have a portal. The staff will request your email address, and may have you complete a consent form. Within 30 minutes, you will receive an invitation with a link to enroll. Just click on the link and follow the instructions to complete your enrollment.
Children under 12 years of age and dependent adults who have a Dignity Health doctor may have a portal with an adult as the proxy. Accounts are not available for adolescents 12 – 17 years of age.
Some of the information in your portal is provided through your medical health record and is viewable by you and authorized staff of Dignity Health facilities. If you would like someone to have access to your records to help manage your care, you can sign a release to authorize a trusted relative to be set up as a proxy.
The enrollment link expires 90 days from the day they are issued. If you are not able to find your invitation or it expired, you may request a new invitation from the staff at the facility that assist with your medical records.
Log in with the email address you provided to the hospital or clinic during your visit. Navigate to “My medical records” or click the “View medical records” icon. Select your region, and you will be prompted to link your accounts and access your medical records.
