This includes educational materials, information on your current medications, allergies, immunizations, health issues, and lab test results. All information is pulled directly from your electronic health records.

You can complete our online form to easily and securely request your records.
Come to our office at 228 N. Cherry Street, Fruita, CO. 81521 to file an in-person Release of Records Authorization Form. We are open from 8 a.m. – 5 p.m., Monday – Friday. We are closed on holidays and weekends.
If you are an outside facility, you can email us a copy of your Authorization to Release Request to RecordsRequest@fhw.org
