Step 2: Enter your date of birth and your unique patient number. Current patients get their unique patient number from their clinic.

Simply call us at 847-729-2188 anytime during business hours and we can verify the payment due, set up payment arrangements, and take your credit card information over the phone. We accept Visa, MasterCard, American Express and Discover.
To mail your payment to us, simply send us the top portion of your billing statement along with a personal check or your credit card information authorizing payment.
