Step 1: From any computer, tablet or smartphone, go to FCI.MyHealthPatientPortal.com and click “Register” on the login page. Step 2: Enter your date of birth and your unique patient number. Current patients get their unique patient number from their clinic. If you forget your patient number, click here to contact your clinic and they will help you.
