A password-protected Web portal that gives you 24-hour access to certain personal health information from your FPG medical record. For urgent requests, phone your doctor’s office.

Billing & Insurance

For your convenience, our central billing office is staffed Monday through Friday from 8 a.m. to 4:30 p.m. You can call the office at 419-557-5530.

Patient Forms

You can download, print and fill out some of your patient registration forms prior to your appointment to reduce wait times.

Advance Directives

Advance directives are legal written documents that indicate what an individual wants regarding care and treatment if he or she is not able to speak for himself or herself. There are three documents: a healthcare power of attorney, a living will and a do-not-resuscitate order. All must be signed by the competent patient and witnessed or notarized.
