How do I sign up for the medical records portal?

The staff members in the Medical Records department will be able to walk you through the process to sign up to the portal. Online self-enrollment is available if you are 18 and over. You will need your medical record number in order to successfully register online. Online self-enrollment is currently not available for caregivers or guardians.

Do Frederick health patients still have access to the NextGen patient portal?

Patients of selected Frederick Health Medical Group specialties will still have access to the NextGen Patient Portal. You may use this portal to access historical information related to care provided by any participating practice.

What time do you call a patient at a hospital?

Patients may receive calls in their rooms from 7 a.m. to 10 p.m. Local calls may be made from the room at any time by dialing 9 and the number. Long distance / toll calls can be placed by dialing 9 and then entering credit card or phone card information.

Visiting Guidelines. Food or refreshments may not be given to a patient without consent of the attending physician or nurse in charge of the patients care. How old do you have to be to have a portal account?

Patients 13-17 may have their own portal account. Patients age 18 and older may grant proxy access to their portal account through an approved Personal Representative Authorization Form, which may be done at any Fairfield Medical Center or Fairfield Healthcare Professional Location.

Where is the FMC portal located?

If you do not have a scheduled appointment and wish to sign up for My FMC Portal, you need to go to the FMC Medical Records Department, which is located on the first floor at 401 N. Ewing St. in Lancaster, Ohio. The hours are 7 a.m.-8:30 p.m.

Does FMC have a password?
