How long are hospital records kept?

All records are maintained, archived and onsite for 10 years following the date of discharge, after which they are destroyed. This applies to both inpatient and outpatient cases that are hospital-based services.

When Do I Receive My Medical Record Information?

GW Hospital has contracted with CIOX Health to process your request for medical records. Medical Record Information is processed and mailed within 7-10 business days. An invoice (if applicable) will accompany the information. Please make all checks payable to:

How to sign up for MyPatientChart?

Self sign-up . Visit MyPatientChart.org and click Sign Up Now. Enter your information and submit the form (which uses a secure third-party identity verification system) to activate your account. Self sign-up is only available for patients age 18+.

How long does a patient chart code last?

Activation code from your provider’s office. Visit MyPatientChart.org and click Have An Activation Code? Enter the code. Your code expires 90 days from date of issue. Follow the instructions to activate your account.

Can a parent access my chart?

Yes. Proxy access and allows a parent or guardian to log into a child or dependent’s personal MyChart account, and then connect to information regarding that patient. Visit MyChart for more information on Proxy access.
