How do I get my medical records from Grandview Medical Center?

Print the Medical Records Request Form, fill it out completely and bring it in person or mail it to Release of Information at Grandview Medical Center along with a copy of your government issued photo ID. Requesting Records on Others Records may be requested by a parent or guardian of a minor child with the exception of psychiatric records.

How do I get information from the previous patient portal?

Information contained in the previous patient portal can still be accessed by contacting your local hospital. Click here for information on how to do that. What is the number to call Grand View Health?

If you need additional information before, during or after your stay, call Patient Relations at 215-453-4975. Grand View Health is committed to patient safety. The most important step you can take is to wash your hands with soap and water or use an alcohol based hand sanitizer.

Spiritual Support. Spiritual comfort in times of illness, injury or the death of a loved one can be an important part of the healing process. The unexpected fears and overwhelming threat of an uncertain future can often send patients and their families in search of spiritual, emotional and physical healing.

Does Grand View Health have outpatient offices?

Their practice is located within Grand View Health and is available to patients 24 hours a day, seven days a week. They only see hospitalized patients and do not have outpatient medical offices. These physicians have strong backgrounds in general and critical care medicine.

Do you bring advance directives to Grand View?

Be sure to bring a copy of your advance healthcare directive with you every time you are admitted to Grand View. Also, give copies of your advance healthcare directive to your doctor and loved ones so your wishes would be known should you be admitted to the hospital unexpectedly. Spiritual Support.

Will my portal information be secure? Yes. MyHealthHome uses a secure, encrypted connection and all health data is stored on a secure server. Personal and medical information in the MyHealthHome portal remains confidential. Only you – and those you authorize – are able to see or gain access to the information in your portal.

How old do you have to be to visit Grandview Medical Center?

Grandview Medical Center is currently allowing one (1) visitor per patient. Visitors must be age 16 or older. No children allowed. All visitors will be required to: Wear a mask while in the hospital – mask should cover both mouth and nose. Social distance while in the hospital.

How many visitors are allowed in NICU?

For the NICU, only one (1) visitor will be allowed.

Requesting Records on Yourself

Records may be requested by a parent or guardian of a minor child with the exception of psychiatric records. Records may be requested by persons holding an active Medical Power of Attorney. Call the Release of Information (ROI) Department to see if you qualify to request records on others.
