How do I apply for the healthwell hotline care manager program?

Apply by phone. Call (800) 675-8416. We strongly encourage providers, advocates and social workers to use the PROVIDER PORTAL to apply so that patients can readily access our hotline care managers. HealthWell reserves the right to change its program in its entirety or with respect to any applicant at any time with or without notice.

What is the Northwell FollowMyHealth patient portal?

The FollowMyHealth Patient Portal works hand in hand with your Northwell account to give you more access to your personal medical records—from Northwell and other medical providers—and more control over your health care. Visit FollowMyHealth to sign up or log in. Some Northwell Health practices are affiliated with other patient portals.

Do I need a patient portal account to re-enroll or apply?

A Patient Portal account is not required to re-enroll or apply for a grant. To check the status of an application or supporting documentation that does not appear when using the portal, please send an email message to Grants@HealthWellFoundation.org.

What is the pharmacy portal or provider portals?

If you are a health care provider’s office, a pharmacy representative or a social worker applying on behalf of your patient, you must use the PHARMACY PORTAL or PROVIDER PORTAL to complete and submit your application.

We offer financial assistance through a number of Disease Funds, with new funds opening every year, so they can get the care they need.

HealthWell helps with treatment for a variety of diseases and is currently accepting applications for grants from our Open Funds. We are always expanding our programs. Please be sure to check back if a disease is not currently listed among our Open Funds or submit a request for a new Disease Fund.

The Disease Fund (s) under which the medication is covered will appear. Please note: To get coverage for a medication, it must be covered under the specific diagnosed disease for which you are applying.

Anyone with the patient’s express permission may apply for a grant on his or her behalf, including specialty, retail and mail-order pharmacists, pharmacy staff or advocates.

How HealthWell Grants Work

HealthWell allocates each patient a grant for a rolling 12 months, after which you or the patient may reapply as long as funding is available. Grant amounts vary by disease state.

Where can I get FollowMyHealth?

You can install the free FollowMyHealth app from the iTunes App Store or Google Play store. Once you have the app, you can choose to specify certain notifications be sent via text message to your mobile phone. Text messages will not contain protected health information.

What is proxy access?

Proxy access enables you to view some of your child’s health information in the portal and, in some instances, communicate through the portal with your child’s healthcare team.
