All information is pulled directly from your electronic health records.
The Holy Cross Medical Group Follow My Health portal is supported directly through your doctor's office. For access to or assistance with the Follow My Health portal, contact your Holy Cross Medical Group doctor’s office.
To enroll in the patient portal, you must receive services at one of our locations. To create an account, you may self-enroll here. Alternatively, you may contact the location where you received care and tell them you would like to make an account. Registration requires an email and a valid ID.
The Health Information Management Services (Medical Records) Department is responsible for managing requests and releasing copies of medical records. Due to the confidential nature of a patient's medical record, Holy Name Medical Center requires ALL requests for the release of medical records be accompanied by a completed, HIPAA-compliant, Authorization for Release of Medical Information Form, signed by the patient or legal representative.
Dates of treatment. Enter the date range, which is located under part 2 on the Authorization for Release of Medical Information Form.
Demographic information of the patient. Include any aliases, married name, and date of birth.

The Follow My Health Portal contains your Holy Cross Medical Group physician office records. Already have a Follow My health account? Access your Holy Cross Medical Group records . The Holy Cross Medical Group Follow My Health portal is supported directly through your doctor's office.
You may see some results before your provider.
If you prefer to hear the results from your provider, you may wait for your provider to contact you. As the patient, you can choose when to view your results in the patient portal. Your provider looks at every result, yet not all results will generate a phone call to discuss.

Due to the sensitive nature of radiology and pathology test results, these are not available on the patient portal and must be obtained through the hospital’s Health Information Department.
RWJBarnabas Health is in the process transitioning all of our hospitals to a centralized patient portal called MyChart where all hospitals’ records will eventually be contained within one system for your convenience. Please be assured that you will always have access to your medical records.
