When you have a planned visit to an INTEGRIS Health facility, you may be contacted to “pre-register” for your service. If you have questions, please contact the Patient Access Center at 405-951-4703. This informs you what rights Oklahoma law gives you to make medical care decisions.

How do I invite someone to join Integris?

Click Invite Someone, complete the form, and click Send Invite. If the person to whom you are granting access is not an INTEGRIS patient, or if you are not sure, please complete the Portal Proxy Request for Adult Form and an account will be created for them.

Does Integris have an app?

INTEGRIS Rx is a free application that connects you to your local INTEGRIS hospital pharmacy. Search for your preferred pharmacy using your ZIP code or GPS location, then save the pharmacy as your favorite to keep connected!

How do I set up Integris and me?

Request an Activation Code Enter your personal information to access your own health records. Once the request is submitted, please allow up to a week for verification and processing. Upon approval, you will receive an email or a letter with your activation code and instructions on how to activate your portal account.

How do I reset my Integris and password?

Please verify your personal information. If you do not remember any of this information, you will have to contact your INTEGRIS and Me help desk at 844-843-1500 to help you regain access to your INTEGRIS and Me account.

What is a DNR form?

A DNR consent form is a form signed by a competent adult person or the patient’s legal representative that authorizes withholding or withdrawal of cardiopulmonary resuscitation (CPR) in case of cardiac or respiratory arrest . It is effective immediately when signed. You or your representative may also choose to limit only certain types of resuscitation, such as intubation, mechanical ventilation or administration of medicines. In Oklahoma, every person must receive full resuscitative efforts in the event of a cardiac or respiratory arrest, unless a DNR consent form has been signed by the patient or the patient’s legal representative or another legal exception applies. Without a DNR consent form or other legal document authorizing the withholding or withdrawal of CPR, your physician may only enter a DNR order when there is documented clear and convincing evidence from your family members, friends or others that you did not desire to have CPR, your physician determines that CPR would be futile or your death is imminent.
