Jackson Health System’s Health Information Management Department maintains medical records for the benefit of the patient. Patients are able to view documents, request appointments, and maintaining personal information on the MyJacksonHealth Patient Portal.
The JCMH Patient Portal gives patients instant access to their health information, including laboratory and radiology results, medications, allergies, demographic information, visit history, discharge/aftercare information, and more.

Enroll in the Patient Portal by either completing the online self-enrollment process or by filling out an enrollment form and returning it to the JCMH registration area, the JCMH Health Information Management department or a JCMH Medical Clinic receptionist.
