What to bring to a dental visit if you don't have insurance?

If you don’t have insurance, please bring proof of your household income (a recent check stub or letter from employer) and cash, check, or credit card to pay for your visit.

Can I telehealth a patient?

We do offer telemedicine. Please call your preferred health center location to inquire or set up an appointment. We can also switch your previously scheduled appointments to a telemedicine appointment if you'd like. Telemedicine is available for many follow-up appointments (i.e., if you have a scheduled check-in for a chronic condition or if you need a visit to get a medication refill), as well as sick visits. If you have a fever, cough, or other symptoms, we can assess those in a telehealth visit without bringing you into the office. You don't need to download anything special to take advantage of this option but you will need a smartphone, tablet, or computer with an internet connection and functioning camera.

Where is the parking lot in Jericho?

There is a municipal parking lot one block west of our office on Jericho Turnpike. Parking is available up to eight hours.

Do you need a referral for a medical exam?

You must have a physician referral-your exam cannot be performed without it .

What is Jericho Road Community Health Center?

Jericho Road Community Health Center (JRCHC) provides primary healthcare and supportive programming to low-income residents and refugees in the City of Buffalo. Jericho Road has a shared history as Jericho Road Family Practice (JRFP) and Jericho Road Ministries (JRM). Started by Dr. Myron and Joyce Glick in 1997 as a small family practice, JRFP has provided safety net healthcare for low-income and refugee community members, targeting areas of concentrated poverty in Buffalo. JRM was founded in 2002 to provide more integrated care through programs that address non-medical, poverty-related barriers. The two organizations merged in 2013 to serve the community more holistically and more sustainably as a Federally Qualified Health Center.
