The JCMH Patient Portal gives patients instant access to their health information, including laboratory and radiology results, medications, allergies, demographic information, visit history, discharge/aftercare information, and more.
Patients can access Health Records from within the Health app and can download their health records by selecting Johnson Memorial Health and authenticating with their MyJMH patient portal username and password. Contact the JMH Release of Information office at 317.736.3573 and request an Invitation to Enroll.
Once you have registered, you will be able to see upcoming JMH appointments, submit changes to your personal information, see you medication list and allergies on record since your last JMH visit, review your hospital visit history, access your discharge summaries and view your lab results. Downloading forms is an important part of your medical records. The patient portal allows you to download important forms whenever they are needed. Parents and guardians can set up minor and/or dependant adult accounts to view and manage their health care information.

Sign up forms are available at any JMHS facility and online (below). They can be turned into any receptionist once completed.
MyJacksonHealth is a private web portal designed for you to access your personal medical information, making it easy to stay connected and manage your health.
During your registration or discharge, simply provide us with your email address. You will receive instructions in an email invitation within 24 hours of enrolling.
Simply click on the link provided in the invitation email to verify your personal information.
You can contact your health provider or hospital for an amendment form.
MyJacksonHealth uses industry-verified security technology to keep your records safe.
You may download any files located in the “Discharge Summary Packet” section of the portal.

Enroll in the Patient Portal by either completing the online self-enrollment process or by filling out an enrollment form and returning it to the JCMH registration area, the JCMH Health Information Management department or a JCMH Medical Clinic receptionist.
