We do NOT issue certified copies of birth certificates (see above).

We are extremely committed to privacy, therefore, you must come in person in order for you to sign an authorization and present proper identification. We cannot release medical record information over the phone.

Bailey Health Center uses Medicat, an electronic medical records system for maintaining student records. Students can find downloadable forms, upload forms into their medical record and print immunizations records from the Patient Portal. Students use their Lafayette credentials to log in.
Bailey Health Center has partnered with CVS Pharmacy to assist us in educating and vaccinating the students on campus. The event is still in the planning stages. Students should look for an announcement in the Lafayette Today. Employee Flu Clinics will be provided by Bailey Health Center Staff on September 28th & 29th.
