How do I request medical records from Lakewood Regional Medical Center?

Request Medical Records At Lakewood Regional Medical Center, we are required to keep your health care information private. To receive copies of your medical records and images, you will need to complete an Authorization to Release Information form. The form must be filled out completely and can be returned either by fax or by mail.

What is the new patient portal for LRH?

Log In On February 1, 2022, Lakeland Regional Health's Primary and Specialty Care services transitioned from FollowMyHealth to a new, upgraded patient portal, called MyChart! *Please note: You can no longer send messages, refill prescriptions, or request appointments for LRH providers using FollowMyHealth.
