Lifecare Medical Associates is located at 1991 Balsley Road, Seneca Falls, NY 13148. Our providers see patients of all ages from newborns to geriatrics. All care is provided by highly credentialed physicians Dr. Sarah Chang, Dr. Grace Freier, Dr. Elizabeth Osborn, Dr. Andrew Reese and Dr. Philipp Wirth.

Primary Care or Specialist Practice’s Patient Portals. Our primary care and specialist practices utilize a separate portal than the hospitals. Please note you must request a portal activation letter from your physician’s office before you can access the patient portal. Please contact your physician’s office for an activation letter.
