How do I get Set up with Lifeline?

The Lifeline team will guide you through the process to help you get set up with solid clinical care. First we send you information to set up your patient portal and to complete the intake documents. At this first phone call our intake team will set up both your outreach call and intake session.

What is the patient portal?

Your patient portal connects you to the LIfeline team. You will have access to the documents you need to complete and a record of them once they are completed. Our Patient Portal is HIPPA Compliant services for you to connect with your counselor or other administrators to help get your needs met!

Does lifeline PCs sell its data?

Lifeline does not sell its data or in anyway other than authorized for business purposes gives the information to 3rd parties. Lifeline PCS seeks your assistance. Lifeline seeks to maintain standards of quality service and to that end, your feedback matters. To complete the survey, follow the link and complete the questions in the survey.
