How do I access my medical records from Maimonides?

Access your personal medical records from Maimonides and other medical providers on your computer or mobile device. FollowMyHealth® is used by hospitals, doctor’s offices, and health networks across the country to provide patients with their personal health records, which enables you to take a proactive role in managing your care.

How do I contact Maimonides virtual care support?

Contact our support team at virtualcaresupport@maimonidesmed.org or call us at (718) 283-2244. If you are the parent or legal guardian of a Maimonides patient who is under the age of 12, you can request proxy access to your child’s health information in the FollowMyHealth® patient portal.

How do I get an invitation to Maimonides Medical Center?

Maimonides patients typically receive an email invitation when they have a visit or procedure at Maimonides. Look for an email invitation from noreply@followmyhealth.com with subject “Invitation for connecting to Maimonides Medical Center”.

How many family members can be at the bedside of a patient?

Two family members and/or legal representatives may be permitted at the bedside of any patient who is actively dying, or whose death is anticipated within less than 24 hours. The patient or the family/caregiver may designate up to two support people.

Can a companion be accompanied to the hospital?

Patients undergoing same-day procedures may be accompanied to the facility by a companion and that companion may remain with the patient through the initial intake. During the procedure, the companion may wait in the designated Surgical Visitors Waiting Area until the patient is discharged, unless allowed by the Medical Center due to the patient’s status or condition.

Can you leave the hospital if you don't adhere to the protocol?

Visitors must remain in the patient’s room for the duration of the visit except for entering and exiting the hospital. Visitors who fail to adhere to this protocol will be asked to leave the Medical Center immediately (NO exceptions) and future visitation within the Medical Center may be restricted.

Can you access your medical records with FollowMyHealth?

With the FollowMyHealth® Patient Portal, you can access your personal medical records—from Maimonides and other medical providers. You may do the same for your children who are younger than 12 and for adult family members with impaired decision-making abilities.

What is the patient relations department?

The Patient Relations Department works to ensure your safety and comfort. Our patient representatives educate you and your family about your rights and responsibilities, help you communicate with hospital staff, and handle care issues such as advance directives. Most patient representatives are bilingual or multilingual with fluency in Hebrew, Yiddish, Urdu, Spanish, Italian, German, Chinese, Arabic, Greek, or Russian. Our patient representatives are available in the emergency room 24/7 and on inpatient units from 8 a.m. – 12 a.m., 7 days a week. You can reach the Patient Relations Department at (718) 283-7212.

What languages are required to provide a copy of the Patient Bill of Rights?

We can provide versions of this notice in Spanish, Russian, Creole, Korean, Chinese, or Italian.

How can I get a copy of my medical record?

Patient information and data may only be released upon receipt of a written request with appropriate patient authorization, valid subpoena, and a court order or as otherwise required by law enforcement.

Your physician may submit a request for your records to the Medical Records Department.

How long will it take to receive the medical records?

Requests for treatments such as going to a follow-up appointment, consultation or another healthcare facility will be expedited.

How can my records be sent to me?

Records can be sent electronically or paper copy via certified or registered mail (i.e., USPS/FEDEX/UPS).

Do I need to come in person to pick up my medical records?

They will be sent as requested in the request form unless you define that you would pick them up in person.

I'm a healthcare proxy. How can I get a copy of my parent's medical record?

Please provide the supporting documentation to the Medical Records Department.

What is an abstract?

An abstract is a summary of the medical documentation required for treatment purposes (continuity of care) which includes, but isn’t limited to, the discharge note/instructions, procedure/operative reports, consultations, history, and physicals, labs, imaging studies, etc.
