Patient Portals contain your online medical records. You may view your records from your doctor or hospital visits, lab results, x-ray reports, and much more. Just sign into the applicable portal link below; each offers a secure connection that requires your personal login to access your medical records.

Hospital & Clinics Portal (Records Oct. 12, 2020 - current)

If you are in need of your medical records that date back prior to October 12, 2020, please call (712) 623-7000 and ask for Medical Records. A representative is available to assist you Monday - Friday from 8:00 a.m. - 4:30 p.m.
