How do I contact Dallas County hospitals security?

You can contact security at (972) 566-4800. The Visitor’s Center, located in the atrium of Building A, can provide a welcome respite for visitors. Feel free to visit us for information on the hospital, Dallas area and common health conditions. Copier and fax machines are also available.

How do I get a birth certificate from Medical City Dallas?

If the record is needed urgently, please note STAT on the request. When a child is born at Medical City Dallas, we submit the Birth Certificate information to the Bureau of Vital Statistics. Certified Birth Certificates can only be obtained through the Bureau of Vital Statistics, which is part of the Texas Department of State Health Services.

Is the my health home portal still available?

The My Health Home Patient Portal is still available to view your health information prior to February 1, 2016. Any new medical information, after February 1, 2016, can be viewed in the all new MyChart by Prime Healthcare . Your personal and medical information in the My Health Home portal will be kept confidential.

Free wireless Internet access is available throughout the facility for our patients and their families.

E-mail a Patient

If you would like to send a quick note to a patient at our facility, email us with your message. Please be sure to include any information required to make sure your message gets to the right person, and our guest relations staff will deliver your email.

Loan Beeper Service

Visitors with a critically ill family member or a child in pediatrics may check out one of our beepers. These keep you in touch with the hospital, yet allow you the freedom to run errands or even go home for a short time. To request a beeper, please contact security at (972) 566-4800 . A refundable deposit fee of $20 is required for this service.

Pastoral Care

A time of illness can be very trying for family or members and friends. For your comfort, the chapel, located in the atrium of Building A, is open to every denomination at all times and staff chaplains are available 24 hours a day. If you would like a chaplain to visit with you, please tell the nurse or call the operator.

MyHealthOne consolidates many common tasks into one secure, easy-to-use online patient portal. It gives you access to most of your medical records on your desktop computer, laptop, tablet or smartphone 24 hours a day.

Access to hard copies of medical records

To obtain a copy of your medical records, please complete, sign and date the medical records release form below and provide a legible copy of your valid photo I.D (e.g. driver’s license, military I.D or state I.D).

Obtaining a Certificate

For parents, or patients over the age of 18, to obtain a Birth Certificate, an application must be submitted to the Bureau of Vital Statistics.

Any patient over the age of 18 can sign up for their own MyChart account. A parent or guardian can request Proxy access for a child. Proxy Access request can be made by emailing mychart@primehealthcare.com

Yes, current MyChart users can download the MyChart mobile app at no cost. The app is available on iPhone and Android devices.

Make an Appointment

Patients are seen by appointment – on the same day the appointment is made if possible. To schedule an appointment, please call (972) 566-6000 during regular business hours.

New Patients

New patients are asked to arrive 15 minutes early to complete paperwork. You can download, print, and complete the forms prior to your appointment. Please bring the completed forms, a list of all medications you are currently taking, recent copies of x-rays, lab results and your insurance card to your appointment.

Billing

We require payment of your co-payment and past-due account balances at the time of service. We accept cash, checks, debit and all major credit cards.

Insurance

We will submit claims to most major insurance carriers. Please bring your insurance card and I.D. with you to every visit so we can ensure that our records are accurate.

Cancellations or Rescheduling

To better serve the needs of all our patients, we ask that you call at least 24 hours in advance to cancel or reschedule appointments. We make every effort to stay on schedule, although emergencies arise.

After-Hours & Emergencies
