You will now have access to Millennium’s Patient Portal, which allows you to have your health information right at your fingertips.   You can send secure messages to your Provider, request prescription refills, find your lab and test results, and much more.   Plus, the Patient Portal is available 24 hours a day, 7 days a week and 365 days a year!

Patient Information

To begin the treatment process, we will need to gather information on the patient including the patient’s medical history, current condition, insurance, allergies and other information. The patient will fill out a “New Patient Information Form” with this information before the appointment.
