All of the information in the Patient Portal comes from your Community Hospital Electronic Health Record.

How do I request access to the MyHealth Portal Portal?

For questions regarding the Patient Portal, please call (970) 644-4002. Parents/guardians can request access to their minor child’s MyHealth Portal Portal, for children under 12 years of age, by clicking on the “I do not have an account” button above. Please provide the information for the child and sign as the parent/guardian.
