How do I connect a portal?

Plug in your Portal TV:

• Plug in the HDMI cable and power adapter, then connect the power adapter to a power source. Your Portal TV will turn on automatically.
• Plug the HDMI cable into an input on your TV. For best results, use HDMI 1.
• Turn on your TV and select the input.

Who can request access to their spouse's health information?

Legal guardians may request access to their ward's health information. Husbands and wives may request access to their spouse's health information. Adults may request access to their elderly parent's health information — with the patient's permission of course. Request Proxy Access.

What is clinical note?

Clinical Notes, the newest My Patient Connect feature, provides access to important information about your office visits and hospitalizations. The information available via Clinical Notes can help you remember your treatment plan and partner with your providers to ensure you have the best outcomes.

Is Concord Hospital a separate portal?

The patient portal for Concord Hospital - Laconia and Concord Hospital - Franklin is separate from My Patient Connect. In the future, the two portals will be combined but for now, patients need to log in to separate portals based on where they receive their care.

What is My Baptist Connect?

The My Baptist Connect website is a safe, private way for Baptist Health patients to stay connected to their personal health information.

How to access my child's medical records?

View your child’s medical records 1 Parents and legal guardians can request access to their child’s (newborn –11 years old) medical information. 2 Adolescents 12-17 years may have their own portal account. 3 Request access to the portal from your Baptist pediatric provider or visit any Baptist Health hospital registration desk. 4 You will be asked to show photo identification as the child’s parent or legal guardian and sign an authorization form. If the legal guardian is not yet established in the patient registration system, you will be asked to visit the Health Information Management (HIM) department at any Baptist Health hospital to show the guardianship court order.

How to enroll in my baptism?

To enroll, all you need is a secure Personal Information Number , or PIN. We’ll send you a secure PIN by mail after your visit — or we can email it to you as a part of your registration process, if you prefer. Once you receive your PIN, use it to complete your registration at MyBaptistConnect.com – the secure link will be listed in your instructions.

How old do you have to be to have a portal account?

Can you review your medical history on Connect?

Our teams are bringing key components of your current medical history into Connect, which you can review with your provider. Rest assured that your doctors will have access to your full record.

Can an adult access another adult's account?

Any adult can be given access to another adult’s account, by granting portal proxy access. The portal proxy does not need to be a patient. The patient will need to complete the Connect Patient Portal Proxy Access Authorization form.

Who can sign up for PatientConnect?

All patients aged 13 and older. A legal guardian may access the adolescent’s record with proper authorization. Learn more about PatientConnect for Teens.

Does Billings Clinic have a bill pay portal?

Billings Clinic is rolling out new, convenient ways for you to pay your bill. Please visit the Bill Pay site for more information about which locations offer the new Online Bill Pay portal. The portal lets you view your bill and account details, lets you set up a payment plan, and view your current and past statements.

Weill Cornell Connect is your patient portal, offering you personalized and secure online access to view portions of your medical records and manage your healthcare.

How to pay Weill Cornell medical bills?

You can pay your Weill Cornell Medicine provider bills online through Weill Cornell Connect. Logging into Weill Cornell Connect also allows you to pay bills from Columbia and NewYork-Presbyterian, if you have received services from them.

Can I use the Weill Cornell app?

You can also use the Weill Cornell Medicine app — available for both Apple and Android devices — to pay your Weill Cornell Medicine services. All electronic fund transfers and payments are compliant with Payment Card Industry (PCI) standards.

Does Weill Cornell Connect require a password?

Your health records are stored behind a firewall to prohibit unauthorized access. In addition, Weill Cornell Connect access requires a username and password that you create yourself.
