How old do you have to be to get a proxy in Missouri?

In accordance with Missouri law, all requests for proxy access to minors between the ages of 13 and 18 will automatically be denied. Minors between the age of 13 and 18 can grant their parents proxy access to their health record, by competing this form.

To enroll, you will need your medical record number (MRN). If you don’t know your medical record number, please call TMH’s Central Registration at 850-431-3042.
