By simply signing up at the clinic front desk, you can have 24-hour access to your medical records on the internet. To register, you will provide an email address and sign a consent form. Next, a temporary password will be e-mailed to you with access instructions.
Our Vital Records Call Center is also available from 8:00 a.m. to 4:00 p.m. to answer your questions at 1-866-534-0051 or 505-827-0121. We only issue certificates for individuals who were born in the state of New Mexico.

A reasonable charge is not more than $30 for the first 15 pages, and $0.25 per page thereafter. Patients may be charged the actual cost of reproduction for electronic records and record formats other than paper, such as x-rays. The board will review the reasonable charge periodically.
Generally, vital records, such as birth, death, marriage, divorce records, are restricted from public access until a statutory period of confidentiality elapses. Until then, New Mexico Vital Statistics Act only allows persons who can demonstrate a direct, tangible interest to obtain vital records.
