My Health Connection is a patient portal service provided for AFM patients by UCHealth, due to our shared electronic medical record system.

How to access my health information on MyHealthConnection?

To allow a family member access to your health information on MyHealthConnection Patient Portal, you will need to fill out the appropriate MyHealthConnection Proxy Access form and have the family member sign it . Bring it to Novant Health UVA Health System Culpeper Hospital's Medical Records department. Once this form is filled out, it will be scanned into your electronic record to show that you granted permission to a proxy. If the family member does not currently have a MyHealthConnection account, then he or she will need to provide an email address to receive portal notifications.

How to change my health password?

A. To change your password, you need to know your old password. Log in to MyHealthConnection, go to "Preferences" and "Change Password." Enter your new password when prompted and select "Submit."

Is MyHealthConnection secure?

A. MyHealthConnection is a secure application. It is important that when you exit MyHealthConnection, you log off and close all browsers that you have open. Please notify us at MyHealthConnection@culpeperhospital.com immediately if you change the email address where you receive portal notifications.

What is the UCHealth app?

Take your medical record with you, thanks to the UCHealth App. The app integrates with My Health Connection to give you more ways to track and interact with your health, including access to your medical record and test results, health news, healthy recipes and exercises, and inspirational patient stories. The UCHealth App is available from the Google Play Store for Android devices (version 5.0 or higher), and via the iTunes store for iPhones (iOS9 or higher) – search “UCHealth.”

Is Estes Park Health still putting your medical information into FollowMyHealth?

Estes Park Health will no longer be putting your new medical information into the FollowMyHealth portal. If you need to view your medical information from prior to November 9, 2019, you may sign into the old FollowMyHealth portal here. This is no longer a live portal with which to communicate to your Estes Park Health physician.
