How do I enroll in the Stamford Hospital Patient Portal?

Your portal enrollment card and medical record number are required to enroll in the Stamford Hospital Patient Portal. If you already have your portal enrollment card and your medical record number, you can self-enroll. If you do not have your portal enrollment card and medical record number, contact the Medical Records Department at (203) 276-7034.

What is the patient portal?

Our patient portal offers a secure and confidential web-based tool for accessing your medical record. Store, Review, Share Health Records: Do you need to print out your medical records? Or did you want to share them with another physician?

How do I self-enroll in the patient portal?

If you already have your portal enrollment card and your medical record number, you can self-enroll. If you do not have your portal enrollment card and medical record number, contact the Medical Records Department at (203) 276-7034. Sign in to our easy-to-use text-only patient portal.

Who do I contact for questions about the patient portal?

If you have any questions about accessing the patient portal, please contact the Health Information Management Department at 203.276.7034. After April 5, 2021 test results will be made available without delay.

Stamford Hospital

Stamford Health Medical Group

You must visit your physician’s office in person to enroll in the Stamford Health Medical Group Patient Portal. Please call for office hours. To find out if your physician is part of the Stamford Health Medical Group, go to our Find a Doctor search.

Stamford Virtual Health Kiosk

In order to enroll, you'll need your First Name, Last Name, Date of Birth and Pin. Your pin will be provided to you at your visit.
