Call Oak Street Health at 888-312-5842! We are always here to help you. Government public health authorities are asking patients to stay away from hospitals unless you are having a medical emergency. 911 is always available, but in these extraordinary times, we ask you to call Oak Street Health first in all but extreme circumstances.

How to log into a patient portal?

To login, begin by clicking PATIENT PORTAL at the top of the web page. Enter user name and password. Click Log in. Along the top and left side of the screen are various options.

How to request labs?

To Request A Lab, Contact the Medical Staff/Physician, Request A Refill. Click on “My Messages”: Click New: To request labs, a prescription refill or if you wish to contact your physician/care team choose “Ask a medical question”: Fill out the necessary information and click “Send”:

Set up your account easily

If you have an email address and you are at least 18 years old, all you have to do is give your email address to a staff member to register. Or call (585) 637-3905.

Change your username

Your email address will be your username, but you can change your username to something different. Just let us know and we’ll update it for you while you’re in the office or call (585) 637-3905. After you’ve changed your username, you’ll get an email with the new username and a temporary password within 24 hours.

Download the Healow app on Google Play or the Apple Store. Once the app is installed and running, it will ask you for a “practice code,” and that code for Oak Orchard Health is ACJFAA.
