Our family practice has six locations, with our central location at 535 Main Street in Olean, NY which includes support services to better meet our patients' needs. Our satellite offices are located in Cuba (2), Ellicottville, Franklinville and Salamanca, NY, all within 12 - 28 miles of our main office.
As of April 1, 2021, NYS says patients “may bring a companion with them to healthcare appointments”. The companion’s need to adhere to the same guidelines as the patient such as wear a face covering, hygiene, temperature screening and symptom screening.
The YourHealth portal allows you to have convenient, easy access to your personal health information anytime and anywhere.
Nobody under the age of 18 will be granted access to the YourHealth Portal.
To change the email address that YourHealth uses for sending activity notifications, password resets, or other communications, click the Profile icon, select Update Profile, enter the new email address and click Submit.
Your laboratory results and radiology reports are available by clicking on the Health Record button from the portal home page. Click Results to view your Laboratory Results or click Reports to view your radiology reports.
In order to access the YourHealth Patient Portal, you must have one of the following browsers installed: Internet Explorer (version 11 or higher), Google Chrome, or Mozilla Firefox. You must also have Adobe Reader installed on your computer to view any of your reports.
HIM will verify your personal information by viewing your driver's license or identification card and proceed to sign you up for the portal. Once the sign-up is complete, you can log into your email for your new YourHealth portal credentials.
If you see incorrect information within your medical history, please notify our Health Information Management department. Phone numbers are listed next to each hospital at the top of the first page.
Because our labs are dedicated to OMG patients, results are very fast , and your physician is able to easily discuss results with a lab specialist. All OMG clinics have on-site labs.
Please contact your provider to get copies of your lab results. You may also request copies of your lab results by completing the patient requesting results from lab form. Bring the completed form to the lab with identification. Please note that Oregon law requires the lab to wait 7 days from the time we receive your request before sending a written copy.
However, some clinics prefer you to schedule an appointment in advance. The few tests that do require an appointment are 2 and 3 hour Glucose Tolerance tests.
We are not accepting walk-ins at all OMG clinics.

OMC MyChart offers a convenient way to pay your bill, update insurance information, and ask questions about your bill. You will also be able to view itemized bills, statements, and payment summaries.
These results will be sent to your OMC MyChart after five (5) business days. The delay is in place to allow time for your provider to review your results and call you before they are placed in your OMC MyChart.
