As of 8/7/2017, OrthoWest, Ltd. was notified of a data breach by CoPilot Provider Support Services. If you received a letter from them about this breach, or have any questions about this matter, you can contact them at 855-205-6948 for more information.
Forms
Prior to your first visit you will need to complete both the Medical History and Demographics forms. You can complete these forms in our waiting room, but you may prefer to fill them out at home, so here are links to the electronic versions of our forms.
Insurance
We participate in most major insurance carriers, including Medicare, Medicaid, BWC, as well as many smaller carriers, so please call us.
Online Appointment Request
Request an appointment online by providing your basic information and preferred appointment times, and a member from our Omaha orthopedic clinic will follow up with you to confirm your appointment. We are also offering Telehealth appointments.
