How do I login to the primary care plus patient portal?

Click the Primary Care Plus patient Portal link here. The patient portal login page will display in a new browser window. Login using the user name and temporary password assigned by the office. Please maintain the temporary password until you create a new password.

How do I sign up for my care plus?

To get started, at your first OHC appointment, tell our team member at the front desk you want to sign up for My Care Plus. They will provide you with information on how to use the program and refer to the steps below. Stop by the front desk at your next appointment and ask to be enrolled.

What is MyChartPLUS?

MyChartPLUS allows you to actively participate in your care, and also alerts you to important health-related reminders to help you better plan the details of your ongoing health care, using your PC, smartphone or tablet.

Patient Forms

You now have immediate access to more information in your MyChartPLUS electronic medical record, including additional clinical results and clinician notes as soon as they’re complete.

Using the access buttons above, download the My Health+ app from the App Store or Google Play, or visit My Health+ on the web (no download required).

Step Two: Login

Login using your existing My Health username and password. If you don’t have an existing My Health account, simply create a My Health+ account in the app or online.
