How do I obtain my medical records from Boulder Medical Center?

To have your medical records released or obtained by Boulder Medical Center, a Medical Record Release Form must be completed and signed by the requesting party or their legally authorized representative. Please obtain the Form below.

What information will be available through the new mybch patient portal?

The majority of information available through the current portal will be available to you through the new MyBCH patient portal. However, some elements of your records, such as the complete history of visits, messages to your care team and notes from your clinic visits, will not be available.

Is the mybch app the same as the Boulder Community Health app?

While both include the same functionality and information, the MyBCH app is a standalone app that features Boulder Community Health branding. We recommend downloading the MyBCH app if you only use Boulder Community Health facilities. What if I have issues with the MyBCH app or the information inside?
