Connect is the patient portal, offering you personalized and secure online access to view portions of your medical records to manage your healthcare. Connect is a single portal for Columbia University Irving Medical Center, NewYork-Presbyterian, and Weill Cornell Medicine.
Connect is a joint effort of Columbia, NewYork-Presbyterian, and Weill Cornell Medicine, so a single Connect account links you to all three institutions. With the Columbia Connect app, you can access convenient services to help manage your health.

Rest assured, your account remains active and you may continue to access your information (link is external and opens in a new window) . No new health information has been added to your FollowMyHealth account after Feb. 1, 2020, and you can no longer connect to your provider through that account.

The health information in your Connect account comes directly from your doctor's electronic medical record. For outpatient records, you can ask your doctor to correct any inaccurate information at your next visit, or send a message through Connect to your provider’s office to request corrections to your health record.
Any adult can be given access to another adult’s account, by granting portal proxy access. The portal proxy does not need to be a patient. The patient will need to complete the Connect Patient Portal Proxy Access Authorization form.
