Welcome to MyDCH, DCH Health System's patient portal. MyDCH Patient Portal is an online tool that goes a step beyond simply displaying information by helping you manage your health care easily and securely.

What can I do through the Cancer Center patient portal?

Through your Cancer Center Patient Portal, you can view your appointments and test results, complete health care assessments, access educational materials and view contact information for your care team from the comfort of your own home. The portal is designed for secure messages between your Provider, the Nurse and you.

How do I complete a self-enrollment request for mydch?

If you provided your email to DCH staff during your visit, you will need to access the MyDCH portal page to complete your self-enrollment access request. Please use the following steps to complete this request:

How to enroll in MyChart?

It's easy to enroll in MyChart. Simply request a MyChart activation code online or during an office visit with any participating practice . Your activation code enables you to log in and create your own user ID and password. If you are not already enrolled after being discharged from the hospital, emergency department, or an outpatient office, you can request an account by calling MyChart Support at 833-254-3600.
