What is the patient portal and how do I use it?

The Patient Portal can be used to view, download or print your visit summaries, laboratory results, and patient education. We recommend entering the Patient Portal via link in the email you receive, which will also contain your username and temporary password.

How to send medical records to a physician?

To send your medical information, use the Send Health Record button. Be sure to read the on-page instructions carefully. If you don’t know the email address, you will need to ask the physician you are trying to send your records to. If you chose a date range in View only records for those dates will be sent.

How to download medical records?

To download your medical record, use the Blue Button. You can save information to an SD card or a thumb drive, and give it to the other practice by hand. You can download data as a PDF and as an XML file. If you chose a date range in View only records for those dates will be downloaded.

How to visit my health record?

After logging into MyHealthRecord.com, select Visit from the menu and pick a visit from the list.

How to fill out a health history form?

Under My Health, select Health History Forms and select a form to fill out. (You can only see the online Health History Forms if your doctor’s office offers them.) Do this for every form you’re asked to complete. Remember, if you don’t have time to finish a form, click Finish Later in the form. This saves your answers for later. Forms that haven’t been finished are marked as being ‘In Progress’ in the Health History Forms list.

Where is Pay My Bill option?

If your provider accepts online payments, you will see a Pay My Bill option in the Navigation bar.

How to find my child's health record?

Click on your name on the main menu. If you have access to your child’s health record you will see their name in the list. If not, please contact the office.

How to make an appointment at Pateint?

You can make an appointment by calling (334) 396-9100 week-days between 7:45 in the morning and 4:00 pm in the evening. Our phone nurses shcedule appointments and answer calls regarding clinical questions. Established patients may also schedule their annual exam through our Pateint Portal. Please arrive on time for your appointment and we will do our very best to see you on time. All new patients should arrive twenty minutes early to complete in processing.

Can a family member be allowed into the exam room?

If the patient requests it, and after an initial private meeting, the doctor may elect to allow a single family member into the exam room. Exceptions to this should be coordinated in advance. Unless there is a documented clinical need patients only will initially be allowed back.
