Get your medical record number (MRN) and activation code by calling the Patient Portal Help Desk at 734-615-0872 (7:30 am - 4 pm). Then create your account at MyUofMHealth.org by clicking the blue "Sign Up Now" button If you forgot your username or password, go to MyUofMHealth.org and click the Forgot Username? or Forgot Password? links.
View your health summary from the MyUofMHealth.org electronic health record Communicate electronically and securely with your medical care team Press the Star icon to add to your My Favorite Links on the home page.

Your MyUofMHealth username must be between five and twenty characters, but cannot include symbols. Once selected, your MyUofMHealth username cannot be changed. Your MyUofMHealth password must be a minimum of eight characters and must contain at least one number and one letter.
