With the arrival of Health Records Online, we will begin the process of sunsetting the previous patient portal system. Patients can continue to access existing health records already loaded into the previous patient portal environment, but other functions will be disabled.

Two Ways to Create Your MyHealthHome Account

Enrollment Invitation – Commonwealth Health will send an email to the email address you provided upon registration to the hospital. Follow the instructions in that message to create your account.
