New Patients

All new patients should plan to arrive at their scheduled appointment time. Please make sure you have all necessary information with you, including:

Subsequent Visits

Please notify our Receptionist of any change in your registration information. Incorrect phone numbers will make it impossible to reach you with test results or to notify you of a change of an appointment time. Incorrect insurance information will delay benefits due to you under your policy.

Occasional Delays

We make every effort to remain on schedule; however, it is sometimes necessary to meet the needs of hospital or emergency patients. We ask for your patience and understanding should your appointment be delayed.

Designate a Key Contact (Friend or Family)

You may want to talk with your family and friends to identify someone who can accompany you to your first visit to help with the completion of paperwork, meet your physician, and take notes about treatment options and future visits.

Insurance

We are eager to offer assistance in processing insurance claims as a courtesy to our patients. In order to do so, you must provide complete and correct insurance information to avoid delays in payment. Claims are filed within two working days from the date of service at no additional cost to you.

Medicare

If your carrier is Medicare Part B we will complete and submit all forms on your behalf.

Secondary Insurance

As a courtesy, we will file to your secondary insurance after the primary carrier has paid, assuming we have all the necessary information at the time of service. If we do not have that information, you will be responsible for the balance due.
