Who owns MyChart?

MyChart is owned and operated by Premier Health and Epic Systems Corporation and is fully compliant with federal and state laws pertaining to your privacy. Your name and email address are treated with the same care and privacy given your health records and will never be sold or leased by MyChart.

Attention Patients

If you are on treatment for an autoimmune disease, with the exception of Rituxan, which has special recommendations to discuss with your physician, we recommend that you get the COVID-19 Booster shot. The booster should be administered NO SOONER than 6 Months after your second original COVID-19 shot.

We are pleased and excited to announce our new check-in process!

In an effort to streamline the often cumbersome process of paperwork, we now offer self-check-in at any of three kiosks located in our waiting room. Our new kiosks will not only help provide a better patient experience for you, but will also reduce the amount of paperwork for both patient and staff.

Same day Pfizer and Moderna vaccines are available Monday-Friday at our Manhattan and Bayside locations. Same day Pfizer booster shot is also available to all our eligible patients.
